By checking this box, I acknowledge and agree to the following terms:

1. Safety and Liability Form: Upon joining, I commit to filling out the Safety and Liability Form before participating in any recorded or live breath practices within Nurtur(her).

2. Potential Health Risks: I have read and understood the potential health risks associated with breathwork. Specifically, I acknowledge that breathwork may not be suitable for individuals with certain conditions, including but not limited to:

   • Severe cardiovascular disorders such as:
     • Uncontrolled high blood pressure/hypertension
     • Aneurysms
     • History of heart attacks
     • Myocarditis
     • Atrial fibrillation
     • Advanced kidney disease and diabetes
   • Contagious diseases, specifically those of the respiratory system
   • Severe psychological imbalances like Schizophrenia and Psychotic breaks
   • Detached retina and glaucoma
   • Epilepsy
   • Asthma
   • Pregnancy
   • Any other medical, psychiatric, or physical condition that may impair or affect the ability to engage in activities involving intense physical and/or emotional release.

3. Medical Consultation: If I have any of the conditions listed above or any doubts regarding conditions not explicitly mentioned, I will consult a physician before continuing with any breathwork activities.

4. Billing Agreement: I understand that I will be charged on a monthly basis. I can cancel my membership at any time, provided I give at least 72 hours' notice before the next payment date. I understand that failing to give the required notice may result in being charged for the upcoming month.

5. Proceeding at Own Risk: I understand the potential risks involved with breathwork, and I choose to proceed at my own risk.
